SAN J O A Q U 1 N Environmental Health Department <br /> AMENDED <br /> 1/6/2020 <br /> Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> PEP BOYS#562 1 1401 S CHEROKEE LN, LODI December 20, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I.CLASS II.or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 118 HSC 25160.2(b)(3)Failed to keep copies of consolidated manifesting receipts for three years. <br /> OBSERVATION: Copies of consolidated manifest receipts for the following hazardous wastes were not found on <br /> site during the inspection: <br /> -50 gallons of hazardous waste with waste code 133 from 1/23/2017 <br /> -92 gallons of hazardous waste with waste code 133 from 4/2/2019. <br /> REGULATION GUIDANCE: The generator shall retain each consolidated manifest receipt for at least three years. <br /> This period of retention is extended automatically during the course of any unresolved enforcement action <br /> regarding the regulated activity or as requested by the department or a certified unified program agency. <br /> CORRECTIVE ACTION: Immediately locate a copy of all consolidated manifest receipts from 1/23/2017 and <br /> 4/2/2019 and maintain them on site. Provide a copy of the consolidated manifest receipts and a corrective action <br /> statement to the San Joaquin County Environmental Health Department. <br /> This is a Class II violation. <br /> 120 CCR 66266.81(a)(4)(B) Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years. <br /> OBSERVATION: Bills of lading for the recycling of lead acid batteries from 12/20/2017 through 10/30/2019 were <br /> not found on site. <br /> REGULATION GUIDANCE: <br /> (A)The generator shall use either the manifest or a bill of lading which fulfills the requirements of Title 13 CCR <br /> section 1161,to record the shipment of spent lead-acid storage batteries to a person who stores the batteries or <br /> who uses, reuses, recycles or reclaims the batteries or their components. <br /> (B)The generator shall retain, at the generator's place of business for at least three years,a legible copy of each <br /> manifest or bill of lading which identifies spent lead-acid storage batteries shipped to a person who stores the <br /> batteries or who uses, reuses, recycles or reclaims the batteries or their components. <br /> CORRECTIVE ACTION: Provide copies of battery core bill of lading records from 12/20/2017 through 10/20/2019 <br /> and a corrective action statement to the San Joaquin County Environmental Health Department within 30 days. <br /> This is a Class II violation. <br /> FA0011020 PR0514495 SCO01 12/20/2019 <br /> EHD 22-02 Rev.9/20/2019 Page 17 of 23 Large Quantity Hazardous Waste Generator OIR <br /> 1868 E.
