F. SITE HEALTH AND SAFETY <br /> Upon approval of the remediation system design, the site-specific health and safety plan(HASP)will <br /> be amended to reflect the planned construction activities <br /> Prior to any construction activities or system installations, various representatives of the client, on- <br /> site personnel, consultant, and contractor will meet at the site for a Contractor Orientation Meeting <br /> (COM) At the COM, consultant personnel will present all pertinent safety issues and Job Safety <br /> Analyses (JSA) forms If applicable, the HASP may be amended in response to questions or changes <br /> which anse during the COM <br /> A representative of AES will act as the Site Safety Officer for the duration of the installation <br /> activities During the installation, a "tailgate" safety meeting (TSM) will be conducted every <br /> morning prior to the start of site activities All consultant and contractor personnel will attend this <br /> meeting Any visiting personnel (client, State, etc ) will be briefed on site safety and health <br /> measures and will be required to sign off on their orientation A copy of the HASP will be <br /> maintained at the site at all times <br /> G. SYSTEM WASTE REMOVAL <br /> The primary waste generated by the remediation system will be hydrocarbon vapors from the SVE <br /> system In addition, small amounts of subsurface fluids may become entrained in the SVE vapor <br /> stream and captured in the moisture separator(5-101) The disposal details for these waste streams <br /> are addressed below <br /> System Emissions <br /> An Authority to Construct(ATC) form must be completed and submitted to the San Joaquin Valley <br /> Air Pollution Control District (SJVAPCD) prior to finalizing the Remediation System Design <br /> Accompanying this form will be a Supplemental Application Form for Soil and Groundwater <br /> Remediation This application will specify the proposed equipment and provide a complete <br /> summary of the operating parameters for the SVE System and ICE Unit <br /> Upon approval of the ATC application by the SJVAPCD, the RAP will be finalized with the State <br /> of California Upon the State's approval, a Permit to Operate Form will be submitted to San Joaquin <br /> Valley UAPCD This form will provide final details of the system operating parameters <br /> Treatment system efficiency will be evaluated on a continual basis In the event that it is determined <br /> the treatment system is not achieving optimal performance,the cause(s)will be identified and system <br /> operation modified accordingly If such modification requires system upgrade and/or expansion <br /> (e g , inadequate coverage of recovery wells warrants adding additional wells to the recovery <br /> V-'1 <br />
