1601 Turnpike <br /> Page 2 of 2 <br /> COMMENTS AND CONDITIONS: <br /> 1. Notify the San Joaquin County Environmental Health Department(EHD)at least 72 <br /> hours prior to commencement of fieldwork. <br /> 2. Submit a boring permit application and$89.00 permit fee to EHD. <br /> 3. Report groundwater samples for 1,2-DCA and EDB at 0.5 micrograms per liter(µg/L), if <br /> possible. <br /> 4. Drilling cuttings generated during the well installations will be stored in Department of <br /> Transportation-approved 55-gallon drums,characterized, and properly.disposed of at an <br /> appropriate landfill. The drums will be removed within 30 days of installation of the <br /> wells. <br /> 5. Complete the fieldwork consisting of well installation,development, surveying,and <br /> initial quarterly groundwater monitoring by November 25,2004. <br /> 6. EHD recommends that total depth measurements be documented for each well during the <br /> quarterly groundwater-monitoring events. <br /> 7.
