Four boxes of Clairol Permanent Root Touch-up, each with <br /> 1. a container of permanent color creme (with ingredient ammonium hydroxide—an <br /> Alkaline), and <br /> 2. a container of activating lotion (with ingredient hydrogen peroxide—an Oxidizer). <br /> 4. Submit a statement to the EHD documenting how the facility managed the 24 bottles of DG Body Nail Polish <br /> Remover and 30 tubes of ADCO BP-300 Non-Skinning Butyl Sealant, labeled as flammable,which were found in <br /> the facility's back storage area. See below for more information: <br /> a. A tote containing 24 bottles of DG Body Nail Polish Remover:Tony stated that the nail polish remover <br /> was on recall and that he was waiting for DG Corporate to inform him whether to continue selling the <br /> item or to dispose of it. When Tony scanned one of the nail polish removers, the hand held scanner <br /> stated "Stop Sale" and to place the nail polish remover into the appropriate environmental bin. <br /> b. A box marked with the word "Caulking" and contained 30 tubes of ADCO BP-300 Non-Skinning Butyl <br /> Sealant, each with a warning label indicating the sealant is flammable:Tony stated the tubes of sealant <br /> have been in the facility since the store opened several years ago. <br /> Dollar General#14661: <br /> 1. Violation 103-Submit documentation that demonstrates to employees which hazardous waste category (i.e. <br /> Toxic, Flammable, Alkaline,Acid, Oxidizer, etc.)the following wastes belong to: <br /> a. A DG Home Cherry Blossom Fragranced Candle <br /> b. A broken wax candle of an unknown brand <br /> 2. Violation 106—Submit a corrective action statement that specifically addresses each of the following <br /> issues/discrepancies observed at the time of inspection: <br /> a. A nearly empty and leaky container of Cranberry Premium (liquid) Hand Soap,without any spill clean-up <br /> material to account for the missing liquid. David Barajas, Store Manager, did not know what happened <br /> to the liquid that was once in the container.According to David, when a product is used by employees to <br /> clean the facility or "store-used," an employee will mark the item with a code that is generated once the <br /> item is taken out of the store's inventory.The Cranberry hand soap was not marked with such a code, <br /> while a nearly empty container of Aloe Cream Soap was marked with a "store-use" code. <br /> b. A nearly empty container of Palmolive Ultra Dish Liquid soap, without any spill clean-up material to <br /> account for the missing liquid.The container was not marked with a "store-use" code. David did not <br /> know what happened to the liquid that was once in the container. <br /> c. When asked how cardboard boxes, which become contaminated from leakage or spillage of a chemical <br /> product, are managed, David stated the cardboard is recycled. <br /> Dollar General#15928: <br /> 1. Violation 106—Submit a corrective action statement that specifically addresses each of the following <br /> issues/discrepancies observed at the time of inspection: <br /> a. An expired box of DG Health Coated Nicotine Gum, with an expiration date of 02/2019, was found for <br /> sale on a shelf in the facility. Sue removed the expired nicotine gum from the sales shelf and placed it <br /> into one of two open unlabeled baskets behind the front register. Sue stated that damaged items are <br /> placed into these two baskets behind the register and are processed once a week on Tuesday. Sue <br /> indicated that once a damaged item is processed it would be placed in a HW tote if needed. <br /> b. A Satin Crush Lipstick was found in one of the open unlabeled basket behind the front register. Sue <br /> stated the basket is used to hold damaged items until they are processed every Tuesday. Sue stated that <br /> because the lipstick appeared to be opened,the lipstick would be damaged out and eventually placed in <br /> a HW tote. <br /> Dollar General#13585: <br /> 3 <br />
