(2) Name, address and telephone number of the <br /> facility; <br /> (3) Date, time and type of incident; <br /> (4) Name and quantity of material(s) involved; <br /> (5) The extent of injuries, if any; <br /> (6) An assessment of actual or potential hazard <br /> to the environment and human health outside <br /> the facility, where this is applicable; and <br /> (7) Estimated quantity and disposition of <br /> recovered material that resulted from the <br /> incident. <br /> C. A written submission shall also be provided within <br /> five days of the time the owner or operator becomes <br /> aware of the circumstances. The written submission <br /> shall contain a description of the noncompliance <br /> and its cause ; the periods of noncompliance <br /> (including exact dates and times) , and if the <br /> noncompliance has not been corrected, the <br /> anticipated time it is expected to continue; and <br /> steps taken or planned to reduce, eliminate and <br /> prevent reoccurrence of the noncompliance. <br /> 13. Other Noncompliance <br /> The owner or operator shall report all other instances of <br /> noncompliance not otherwise required to be reported <br /> within 15 days after discovery of noncompliance . The <br /> reports shall contain the information listed in <br /> II.G.12.C. above. <br /> 14. Other information <br /> The owner or operator shall promptly submit all facts or <br /> information which have been omitted or which correct <br /> information in the permit application or any other report <br /> submitted to the Department. <br /> H. Signatory Requirement <br /> All reports or other information requested by the Department . <br /> shall be signed by the owner or operator. For a municipality <br /> or other public agency, a principal executive officer or <br /> ranking official shall sign the reports or other requested <br /> information.
