"I certify under penalty of law that this document <br /> and all attachments were prepared under my <br /> direction or supervision in accordance with a <br /> system designed to assure that qualified personnel <br /> properly gather and evaluate the information <br /> submitted. Based on my inquiry of the person or <br /> persons who manage the system, or those persons <br /> directly responsible for gathering the information, <br /> the information submitted is , to the best of my <br /> knowledge and belief, true, accurate, and complete. <br /> I am aware that there are significant penalties for <br /> submitting false information, including the <br /> possibility of fine and imprisonment for knowing <br /> violations. " <br /> I. Certification of Construction <br /> The owner or operator may not commence treatment, storage or <br /> transfer of hazardous wastes at the facility or modified <br /> portion of the facility until: <br /> 1. The owner or operator has submitted to the Department, by <br /> certified mail or hand delivery, a letter signed by the <br /> owner or operator and an appropriate engineer registered <br /> in California stating that the facility has been <br /> constructed in compliance with the permit; and <br /> 2. The Department has inspected the constructed facility and <br /> finds it is in compliance with the conditions of the <br /> permit; or <br /> 3. The Department has either waived the inspection or has <br /> not within 15 days notified the owner or operator of its <br /> intent to inspect. <br /> J. Waste Minimization Certification <br /> The owner or operator shall retain original signed copies for <br /> at least three years from the date of certification of the <br /> following statement on waste minimization. <br /> "I hereby certify under penalty of law that <br /> personnel under my direction and supervision at <br /> this facility are undertaking specific steps in <br /> accordance with a program in place to minimize the <br /> amount and toxicity of hazardous wastes generated <br /> at this facility to a degree economically <br /> practicable and that the method utilized for the <br /> treatment, storage, or disposal of hazardous wastes <br /> is the practicable method currently available to <br /> this facility which minimizes the present and <br /> future threat to human health and the environment. <br /> I am aware that there are significant penalties for <br /> -8- <br />
