Failed to train personnel within 6 months of hire or taking a new position. Hazardous waste <br />training records incomplete or not found. A person, trained in hazardous waste management <br />procedures, within 6 months of hire, shall teach facility personnel hazardous waste management <br />procedures (including contingency/ emergency action plan implementation). Refresher annual training <br />program must be designed to ensure all employees are able to respond effectively to emergencies: <br />procedures, equipment, systems, including: <br />-inspecting, repairing, replacing emergency & monitoring equipment <br />-key parameters for automatic waste feed cut-off systems <br />-communications or alarm systems, operations shutdown <br />-response to fires or explosions, to ground -water contamination incidents <br />Owner/ operator shall keep records of training (for a minimum of 3 years when the employee left the <br />company) with the following information: <br />-employee name, job title, job description (skills, education, duties), training description <br />Submit training record with the above information to SJC EHD. <br />132. Failed to maintain a log for daily tank inspections. No daily inspection logs can be found for the <br />used oil tank. Owner/ operator shall inspect daily on operating days: overfills, spill control equipment for <br />proper function, corrosion protection, release of waste, gauges, leak -detection equipment, and <br />secondary containment area for signs of erosions or releases. Logs must be retained on site and <br />available for inspectors. Correct by maintaining a log that verifies an operator/ facility's representative is <br />inspecting used oil tank on a daily operating basis. Submit daily tank inspection log template to SJC <br />EHD. <br />133. A hazardous waste tank assessment was not available on site for the outdoor 500 gallon metal <br />used oil tank. A hazardous waste generator shall obtain and keep on site a written assessment for each <br />hazardous waste tank at the facility. This assessment shall be reviewed and certified by an independent, <br />qualified California -registered professional engineer, whom shall attest to the tank system's integrity. <br />The assessment shall determine that the tank system is adequately designed and has sufficient <br />structural strength and compatibility with the waste stored, and shall be written in accordance with Title <br />22 regulations. Immediately obtain written tank assessments for used oil tank at this facility in <br />accordance with Title 22 regulations, maintain a copy on site at this facility and submit a copy of the <br />certified assessment to SJC EHD. Submit a statement how this would be completed.
