Employee training records <br />shall include: <br />1. the job title for each position at the facility related to hazardous waste management, and the name of each <br />employee filling each job; <br />2. written job description for each position listed above; <br />3. written description of the type and amount of both introductory and continuing training that will be given to each <br />person filing positions listed above; <br />4. records that document that the training has been given to and completed by facility personnel. <br />Training records on current employees shall be kept until closure of the facility. Training records on former employees <br />shall be kept for at least three years from the date the employee last worked at the facility. Ensure that employee <br />training is properly documented. Submit proof of correction to the EHD. <br />This is a minor violation. <br />115 <br />CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br />1) No uniform manifests for the disposal of hazardous waste for the last three years (2013, 2014, and January to June <br />2015) were found on site, the manifests available onsite were dated between June 2015 and August 2015. Hazardous <br />waste generators shall retain copies of all manifests signed off by the disposal facility on site for three years and have <br />them readily available for review. Immediately locate a copy of all manifests for the last three years, maintain them on <br />site, and submit copies to the EHD. <br />2) The reviewed uniform manifests (26 manifests in total) generated between June 2015 and August 2015 were found <br />without signed off copies from the destination facility. Hazardous waste generators shall retain copies of all manifests <br />signed off by the destination facility on site for three years and have them readily available for review. If the generator <br />did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the <br />generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the <br />generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of <br />the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that <br />the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a <br />legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the <br />hazardous waste and the results of those efforts. Exception reports shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC.
