Hazardous waste generators shall retain copies of all manifests <br />signed off by the destination facility on site for three years and have them readily available for review. If the generator <br />did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the <br />generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the <br />generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of <br />the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that <br />the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a <br />legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the <br />hazardous waste and the results of those efforts. Exception reports shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. Submit to <br />the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC. <br />This is a Class II violation. <br />301 <br />CCR 66265.31 Facility not maintained to minimize the release of a hazardous waste. <br />1) Metal fines were noted on the floor around the metal cutting equipment located in the tooling room. <br />2) Metal fines and metal shavings mixed with lubricating oils were noted on and around the metal cutting equipment <br />located in the "new tooling" area. <br />Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous <br />waste to air, soil, or surface water which could threaten human health or the environment. Immediately repair all leaks <br />and staining, and manage according to Title 22 hazardous waste regulations. Submit a statement and supporting <br />documentation explaining how these wastes were managed. <br />This is a minor violation. <br />302 <br />CCR 66265.32 Failed to maintain required emergency equipment. <br />The onsite eye wash station, located in the "tooling" area, was not maintained. All facilities shall be equipped with an <br />internal communication or alarm system, a device capable of calling outside emergency help, spill control equipment, <br />and decontamination equipment at all times.
