Hazardous Waste Documentation, Continued <br />Hazardous When a StrongPak technician makes a pick-up at the store, the StrongPak <br />Waste technician is required to leave certain documentation with the store. After <br />Manifests each pick-up, the StrongPak technician will leave a Hazardous Waste <br />and LDRs Manifest and an LDR form. The Manager -on -Duty, as well as the StrongPak <br />technician, is required to sign the manifest and the LDR. Additional <br />instructions for completing a manifest and what must be reviewed prior to <br />signing it can be found in the EH&S Program Binder. <br />NOTE: Do NOT let the StrongPak technician leave the store without getting a <br />copy of these signed documents. If the wet photo lab is serviced by PSC, <br />they must also leave a manifest after each visit. <br />Another copy of the manifest should come to the store by mail. This copy <br />should have 3 signatures on it (lines 15, 17, and 20). If the store does not <br />receive a signed copy of the manifest in the mail within 35 days of pick up, <br />the Store Manager must promptly call StrongPak at 770-409-1500 (or PSC at <br />866-617-1766). <br />All copies of each manifest and each LDR must be kept in the EH&S <br />Program Binder for at least three years. <br />Hazardous Most retail stores in California are required to prepare and submit a HMBP to <br />Materials the local regulatory agency. A HMBP provides detailed information on the <br />Business Plan storage of hazardous materials at the store and includes important <br />(HMBP) emergency preparedness information. 3E Company prepares and submits all <br />required HMBPs on behalf of CVS/pharmacy stores in California. You must <br />be familiar with the contents of your HMBP and ensure that changes in store <br />operations or personnel (management) are communicated to 3E Company <br />promptly.
