What is the name of the analytical laboratory identified on page three of the <br /> application? <br /> 2. If contaminated soil is encountered at this location it can not be transported to <br /> the Sacramento Landfill. If contaminated soil is to be hauled offsite, please <br /> identify a permitted facility that can accept contaminated soil. <br /> 3. Page six needs to be signed by Pete Grafigna acknowledging responsibility <br /> for additional billing for PHS-EHD staff time if necessary. <br /> 4. You need to submit a current workers comp. certificate. <br /> 5. You need to submit two additional closure applications, only one was <br /> submitted. <br /> 6.
