Department of Page of <br />SAN JOAQLA -Policy/Procedure No. PLANT MAINTENOE 3 9 <br />Effective Date Dale Replaces <br />GENERAL HOSPITAL1-7-93 <br />Titie of Poiicy/Procedure <br />HAZARDOUS MATERIAL RELEASE PROCEDURES <br />140TIFICATION PROCEDURES - INTERNAL <br />Persons reporting a hazardous material spill or a threatened <br />release must do so by dialing #99 and providing the following <br />information: <br />1. Their name and extension from which they are calling. <br />2. The location and nature of the emergency. <br />3. -The type of emergency, (i.e., spill, fire, etc.). <br />4. The type and quantity of material involved. <br />The Switchboard Operator will notify the Hazardous Materials <br />Coordinator, other Department Heads and Administrative personnel <br />for activation of the Emergency Release Response Plan. <br />In the event of a fire, explosion or facility -wide evacuation, <br />refer to the "Fire -Safety and Disaster Preparedness Manual". <br />NOTIFICATIO14 PROCEDURES - EXTERNAL <br />The Hazardous Materials Coordinator shall, upon notification of a <br />release or threatened release, do the following: <br />1. Call emergency fire and police on 911. <br />2. Call the San Joaquin County office of Emergency Services at <br />468-3962. <br />:4. Provide the following information to the above agencies: <br />(a) The exact location of the release or threatened <br />release; <br />(b) His/her name; <br />(c) The hazardous materials involved; <br />(d) An estimate of the quantity involved; and <br />(e) If known, the potential hazards presented by the <br />material involved. <br />56.Y <br />
