SAN JOAOUIN <br />Department of <br />PLANT MAINTENANCE <br />Page of <br />4 9 <br />GENERAL HOSPITAL <br />Policy/Procedure No. <br />Effective Date <br />Date Replaces <br />1 <br />1-7-93 <br />Title of Policy/Procedure <br />HAZARDOUS MATERIAL RELEASE PROCEDURES <br />The Hazardous Materials Coordinator shall make additional <br />notifications as required, should the spill migrate offsite via <br />storm drain, sewer, surface conveyance, air, etc., and complete the <br />Hazardous Material Release Report Form as applicable. <br />If the hazardous material is an acutely toxic material or a <br />hazardous substance under CERCLA (such as Ethylene Oxide) in a <br />reportable quantity and affecting areas outside the hospital, the <br />Hazardous Materials Coordinator must make additional notifications <br />as follows: <br />1. Local Emergency Planning Committee <br />2. State Emergency Response Commission <br />3. Provide the following information to the above agencies:* <br />(in addition to (a) - (e) above) <br />(a) Whether the substance is an extremely hazardous <br />substance; <br />(b) The time and duration of the release; <br />(c) The medium or media into which the release occurred; <br />(d) Any known or anticipated acute or chronic health risks <br />associated with the emergency and, where appropriate, <br />advice regarding medical attention; <br />(e) Proper precautions to take as a result of a release, <br />including evacuation. <br />As soon as practicable, a written followup emergency notice must be <br />made, updating the information provided and including actions taken <br />to respond to and contain the release. <br />EVACUATION PROCEDURES <br />In the event of a hazardous materials release or a chemical spill, <br />the Department Head or his/her representative will direct personnel <br />to a safe assembly area by verbal commands and/or the public <br />address system using established evacuation routes. If patient <br />evacuation is required, refer to the "Fire -Safety and Disaster <br />Preparedness Manual". <br />'56'' <br />
