According to the designated operator inspection report dated 9-6-2016, the system is"missing alarm <br /> history due ISD upgrade." <br /> On the 7/14/2017 designated operator monthly inspection report, Randall Kirby, designated operator, noted a'cold <br /> start(7-7-2017)due to ISD software upgrade. Veeder-Root still needs testing for functionality. Work done by <br /> Versaras Testing/Repair." <br /> Emergency repairs relating to LIST system monitoring and release detection may be performed by a properly <br /> licensed,trained, and certified contractor pursuant to the EHD's policy and a permit obtained within one business <br /> day from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to begining <br /> work. Immediately obtain a permit from the EHD for these repairs. A"failure to obtain a permit"penalty applies. <br /> Submit proof of correction to the EHD. <br /> This is a repeat violation, Class IL <br /> 207 HSC 25294 Failed to record unauthorized release from primary containment. <br /> On the 12/5/2017 monthly designated operator(DO)inspection report, Randall Kirby, DO, noted that"UDC 5/6 <br /> contain fluid and appears to be leaking gas. Dispenser has been shut down+shear valve have been tripped. <br /> Service work is scheduled for tomorrow." <br /> On 1/23/2017, John Alaniz, inspector, observed fuel leaking from the 1/2 under dispenser containment unit. <br /> While on site,there was no documentation found showing that either of these releases were recorded. Any release <br /> from the primary containment that does not escape the secondary containment and was cleaned up within eight <br /> hours of being detected shall be recorded with the following information: <br /> 1. Operator's name and phone number <br /> 2. List of types,quantities, and concentrations of hazardous substances released <br /> 3. Description of actions taken to control and clean the release <br /> 4. Method and location of disposal of the released substances <br /> 5. Description of actions taken to repair and prevent future releases <br /> 6. Description of method used to reactivate interstitial monitoring after repair or replacement of primary <br /> containment <br /> Immediately record this release with the information required, and maintain it on site with the facility's monitoring <br /> records. Submit a copy of the recordable release statement to the EHD. <br /> This is a Class 11 violation. <br /> 215 CCR 2712(1) Failure to retain a copy of the permit to operate at the facility. <br /> A copy of the UST operating permit was not found on site. A copy of the UST operating permit and all attachments <br /> and conditions shall be retained at the facility.
