The Veeder Root system appears to have been upgraded in September 2016 and a permit was not obtained from the EHD. According to the designated operator inspection report dated 9-6-2016, the system is"missing alarm history due ISD upgrade." On the 7/14/2017 designated operator monthly inspection report, Randall Kirby, designated operator, noted a'cold start(7-7-2017)due to ISD software upgrade. Veeder-Root still needs testing for functionality. Work done by Versaras Testing/Repair." Emergency repairs relating to LIST system monitoring and release detection may be performed by a properly licensed,trained, and certified contractor pursuant to the EHD's policy and a permit obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to begining work. Immediately obtain a permit from the EHD for these repairs. A"failure to obtain a permit"penalty applies. Submit proof of correction to the EHD. This is a repeat violation, Class IL 207 HSC 25294 Failed to record unauthorized release from primary containment. On the 12/5/2017 monthly designated operator(DO)inspection report, Randall Kirby, DO, noted that"UDC 5/6 contain fluid and appears to be leaking gas. Dispenser has been shut down+shear valve have been tripped. Service work is scheduled for tomorrow." On 1/23/2017, John Alaniz, inspector, observed fuel leaking from the 1/2 under dispenser containment unit. While on site,there was no documentation found showing that either of these releases were recorded. Any release from the primary containment that does not escape the secondary containment and was cleaned up within eight hours of being detected shall be recorded with the following information: 1. Operator's name and phone number 2. List of types,quantities, and concentrations of hazardous substances released 3. Description of actions taken to control and clean the release 4. Method and location of disposal of the released substances 5. Description of actions taken to repair and prevent future releases 6. Description of method used to reactivate interstitial monitoring after repair or replacement of primary containment Immediately record this release with the information required, and maintain it on site with the facility's monitoring records. Submit a copy of the recordable release statement to the EHD. This is a Class 11 violation. 215 CCR 2712(1) Failure to retain a copy of the permit to operate at the facility. A copy of the UST operating permit was not found on site. A copy of the UST operating permit and all attachments and conditions shall be retained at the facility. Immediately obtain a copy of the permit and retain it on site. This is a Class 11 violation.
