Permits were not obtained from the EHD for <br />either of these repairs. Emergency repairs relating to UST system monitoring and release detection may be performed <br />by a properly licensed, trained, and certified contractor pursuant to the EHD'S policy and a permit obtained within one <br />business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to <br />beginning work. Immediately obtain a permit from the EHD for the 10/29/13 cold start and the 87 leak detector <br />replacement. A "Failure to Obtain a Permit" penalty applies. This is a Class II violation. <br />201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br />Maintenance and monitoring records for the last three years were not found on site. Records of all alarms and the <br />response to these alarms was not found on site. The site is keeping alarm logs, but these logs are incomplete and do <br />not list all of the alarms for the site. The alarm logs for November 2013 through September 2014 only included sensor <br />out alarms on November 14, 15, 16, 17, 19, and 21, and do not state which sensor was in alarm. The other days on the <br />alarm logs are either blank or "all functions normal'. These records shall be maintained on site for at least three years. <br />Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and <br />maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the <br />field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) <br />results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring <br />records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II. <br />204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br />An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible <br />on site at all times. Immediately update the monitoring plans in CERS and retain a current, approved copy on site.
