The designated <br />operator failed to check and note the correct test dates for the annual monitoring certification testing and the secondary <br />containment testing on the November 2013 through July 2014 designated operator monthly inspection reports. These <br />nine inspection reports all note that test results are needed. The dates are listed on the August 2014 and September <br />2014 inspection reports. During the monthly inspection, the designated operator shall check that all required testing and <br />maintenance for the UST system have been completed and document the dates they were done. Ensure that <br />designated operators performing monthly inspections at this facility are including all of the required information on the <br />reports. This is a repeat violation, Class II. <br />The designated operator failed to document all the alarms from the alarm history, and to attach an alarm history, on the <br />November 2013 through September 2014 designated operator monthly inspection reports, and failed to check that all <br />alarms were responded to appropriately. The alarm history printed by the technician at the time of inspection only goes <br />back to the cold start performed on November 25, 2013. Alarms prior to the cold start were not available. The <br />designated operator is not printing out an alarm history, but is instead using alarm printouts kept by the site from alarms <br />throughout the month. These records are incomplete. All sensors were set into alarm on 11/25/13 and 1/22/14; the <br />designated operator monthly reports for November and January note only the L4 alarms. The November report did not <br />include the date of the alarm. Neither report indicated the type of alarm (sensor out, fuel, etc.). The missing alarms <br />from the alarm history provided at the time of inspection also include: C1 (UDC 1/2) fuel alarms on 1/30/14 and 4/25/14, <br />and L4 (91 annular sensor) sensor out alarms on 8/1/14, 8/3/14, 9/14/14. The missing alarms from the site alarm logs <br />include sensor out alarms on November 14, 15, 16, 17, 19, and 21, 2013. The alarm logs do not state which sensor <br />was in alarm. No alarms are noted on the December 2013, and February 2014 through September 2014 monthly <br />inspection reports. During the monthly inspection, the designated operator shall review the alarm history for the <br />previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm <br />history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators <br />performing monthly inspections at this facility are including all of the required information on the reports. This is a repeal <br />violation, Class II. <br />207 Designated operator monthly inspection reports not maintained on site for at least 12 months. The September 2013 and <br />October 2013 designated operator monthly inspection reports were not found on site. Designated operator monthly <br />inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the <br />previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies to the <br />EHD. This is a repeat violation, Class II. <br />208 Designated operator employee training not performed or log not kept. The designated operator employee training for <br />Rajinder Kaur was not performed. The only training record found on site was for Rupinder Kaur, dated August 14, 2014. <br />The designated operator shall train facility employees for which he or she is responsible in the proper operation and <br />maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the <br />training records to the EHD.
