0 0 <br />a. All valid, current California UST certificates issued by the International Code <br />Council (ICC), that are applicable to the work being performed, and <br />b. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced. <br />and <br />c. Each person installing or testing any component must hold a certificate of training <br />from the manufacturer of that component. <br />4. All components installed must be certified by an independent testing organization <br />and bear the appropriate mark as required by the independent testing <br />organization. <br />5. A tank integrity test must be performed by a certified tank tester on the regular (87) <br />tank after concrete has been poured and results received by this office within 7 days of <br />completion of work. <br />6. Complete and submit a copy of the Underground Storage Tank Certification of <br />Installation/Modification (Former Form C) for the regular (87), premium (91), and <br />Diesel (E) tanks no later than 30 days after work is completed. <br />8. This permit expires 180 days from the approval date. <br />9. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work.
