The following is an itemized list of underground storage tank violations that have not been addressed for DIAMOND GAS AND FOOD MART as of May11, 2020 . Open violations from June 04 , 2019 inspection Violation #124 - " Overfill Prevention Equipment Inspection Report Form" not submitted within 30 days of inspection . Overfill prevention equipment inspection report was performed on 10/9/18 . An Overfill prevention equipment inspection report form was submitted to the EHD in January 2019 , over two months late . A copy of the test report must be submitted within 30 days of the test . Ensure test results are submitted within 30 days from the day of testing . ❑ This violation was corrected ❑ This violation will be corrected by (date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #201 - Failure to maintain monitoring , maintenance , and follow up action records . Maintenance and monitoring records for the last three years were not found on site . Overfill prevention equipment inspection results , Record for L4 - 89 STp fuel alarm on 6/16/18 . These records shall be maintained on site for at least three years . Monitoring records include : ( 1 ) date and time of all monitoring or sampling ; (2 ) monitoring equipment calibration and maintenance records ; (3 ) results of any visual observations ; (4 ) results of sample analysis performed a lab or in the field ; (5 ) logs of all readings of gauges or other monitoring equipment, ground water elevations , or other test results ; (6 ) results of inventory readings and reconciliations . Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD . ❑ This violation was corrected ❑ This violation will be corrected by (date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #202 - Failure to maintain the " Facility Employee Training Certificate" on-site (after October 1 , 2018 ) . " Facility Employee Training Certificate" dated 2/28/19 was found on site without the signature from the Designated Operator, Furhtermore Noorullah Fnu and Manpreet Singh started working in the facility in May 2019 and April 2019 and are yet to be trained byt the DO . All site employees must be trained prior to taking responsibility as a facility employee if after October 1 , 2018 , or within 30 days of hire if prior to October 1 , 2018 . The " Facility Employee Training Certificate" shall contain accurate information for all employees trained . Update the " Facility Employee Training Certificate" to contain the accurate information and submit a copy of the updated form to the EHD . Page 2 of 4
