Environmental Health Department <br />Underground Storage Tank Program Inspection Report <br />Date: <br />April 10, 2019 <br /> Facility Address: <br /> 14000 E HWY 88 , LOCKEFORD <br /> Facility Name: <br /> H&S Energy Products,#3038 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 214 CCR 2712 Failure to comply with UST operating permit conditions. <br />A work order B46999 from 3/18/19 provided to the inspector stated "L7: Dispenser 1-2 dispenser pan sensor out <br />alarm Resolution replaced 208 sensor site back up all functions normal." EHD was not notified of the replacement of <br />the sensor. <br /> The following conditions are required to meet No Permit Required requirements: <br />1. Email EHD a scope of work by emailing to UST@sjcehd.com. <br />2. Notify the EHD of the time and date of the repair work by emailing to UST@sjcehd.com <br />or fax to (209) 468-3433. An EHD inspector may stop by the facility to inspect the repair and verify the technician’s <br />certifications. <br />3. The service company must install the part and test the system to verify functionality. <br />4. The service company must provide the following documentation to the tank owner/operator for inspector review <br />during annual UST inspection: <br /> a. Description of part(s) installed <br /> b. Name & UST ICC # of Service technician doing the work (manufacturer certification proof) <br /> c. Testing done to verify functionality of the repair <br />Provide proof of functionality verification testing conducted at the time of sensor replacement. <br />Emergency repairs relating to UST system monitoring and release detection may be performed by a properly <br />licensed, trained, and certified contractor pursuant to the EHD'S policy and a notification shall be submitted or a <br />permit (if required) shall be obtained within one business day from the EHD. All other repairs must have a permit <br />submitted to and approved by the EHD prior to beginning work. Immediately obtain a permit from the EHD for the <br />replacement. A “Failure to Obtain a Permit” penalty applies. <br />This is a Class II violation. <br /> 302 CCR 2630(d), 2641(a) Monitoring equipment is not installed or maintained to detect a leak at the earliest <br />opportunity. <br />The 91 product STP sump sensor was sitting at the top of the saddle and not located to detect a leak at the earliest <br />opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible <br />opportunity. The sensor 205 did not have a long enough wire to be placed at the side of the saddle. The service <br />technician on site replaced the 205 sensor with a 208 sensor and relocated to the lowest point of the sump. <br />Because the sensor was not a like for like replacement, a permit must be obtained from EHD.
