This resulted in multiple permits being issued against one fee which <br /> required computer and accounting corrections. A charge of 0.8 hours was added to <br /> your account for these adjustments. <br /> PHS-EHD charges a minimum of 3 hours for call back time which is overtime work that <br /> is not done following the employees assigned work time. The 201 Main Street site <br /> inspection was on Sunday, March 23, 1997, and a minimum of 3 hours was charged for <br /> this date, as well as an additional 3 hour charge for Saturday, March 22, 1997 for the <br /> 501 Weber Street site. <br /> The 501 Weber Street site experienced some of the same incomplete or incorrect <br /> submittal problems, in addition to an owner search conducted by Mr. Infurna at the <br /> County Assessors office. Much clarification was needed to identify which parcel the <br /> bore holes were to actually be drilled on and verifying the property owner. As you may <br /> now realize, the "excessive" review time you referred to isn't review time only, but <br /> includes many telephone calls and other contacts to complete the paperwork required <br /> to issue the permits. <br /> On May 15, 1997, the reports for both of the sites were received at PHS-EHD. PHS- <br /> EHD policy requires staff to review the data and evaluate the findings in order to <br /> determine if further investigation or remediation is warranted.
