The permit holder must notify (the local a c within 30 days of any changes to the <br />monitoring procedures, unless required to obtain approval be ore making the change. <br />Required by Sections 2632(d) and 2641(h) CCR <br />Facility Name: Pacific Pride Card Lock Facility <br />351 N. Beckman Rd.., Lodi, CA 95240 <br />Facility Address: <br />1. If an unauthorized release occurs, how will the hazardous substance be cleaned up? <br />Note: If released hazardous substances reach the environment, increase the fire or <br />explosion hazard, are not cleaned up from,the secondary containment within 8 hours, <br />or deteriorate the secondary containment, the local a enc) must be notified within 24 <br />hours. <br />Any spill will be quickly contained withabsorbant pads or bulk <br />absorbants that are kept in our shop. After containme material <br />will be swept up and placed in a metal resealable barrel'. -to await <br />ick u and tx'ans ortation to a ro er facility. Ths barrel. will <br />be properly labeled. <br />2. Describe the proposed methods and equipment to be used for removing and properly <br />disposing of any hazardous substances. <br />Brooms, la.stic shovels and environmentall friendl absorbants <br />will be used. <br />3. Describe the location and availability of the required cleanup equipment in item 2 above. <br />All of the eaRiRmtn,t is i_n our shop and our office warehouse <br />imediately adjacent to the card lock facility. <br />4. Describe the maintenance schedule for the cleanup equipment. <br />The equipment is routinely inspected at least once a week - <br />5.
