History also shows T1 sudden loss and probe out alarms and low temperature <br />warning on 2-1-08 caused during tank repairs using ticket #T-5029 (documentation not posted yet). <br />Item #10 - Documentation has been posted for SB989 secondary containment testing performed on 7-18-05. <br />Item #14 - New spill bucket was tested and new TLS 350 was certified on 1-28-08 - documentation is not yet available <br />(ticket #LT -5029 applies). <br />IDocumentation is not available for L4 alarm on 1-8-08 caused when fuel line to engine burst and flooded trench. <br />Documentation is not available pertaining to L2 alarm on 8-3-07 or L3 alarm on 7-31-07. <br />Documentation is not yet available for alarms referenced on previous report (1-15-08 & 1-23-08) caused during repairs on <br />ticket #LT -5029. <br />Per Cecilia Q AT&T Dallas, ticket #LT -5029 has not been closed yet because modem line has not been installed to the <br />IObtain and provide documentation for: <br />- L2 alarm activity on 8-3-07. <br />- L3 alarm activity on 7-31-07. <br />- L4 alarm activity on 1-8-08. <br />- TLS installation and certification. <br />- spill bucket installation and testing. <br />- annular riser installation. <br />Instructions: <br />Monthly visual inspection of the UST system must be conducted by a Designated UST Operator, who <br />possesses a current California UST System Operator certification issued by the International Code Council. <br />A copy of this monthly visual inspection checklist must be provided to the UST Owner or Operator. <br />The Designated UST Operator must alert the UST Owner or Operator of any condition discovered during the monthly visual <br />inspection that may require follow-up actions. <br />The UST Owner or Operator must maintain a copy of this monthly visual inspection checklist and all attachments for the <br />previous 12 months. The records must be maintained on-site or, if approved by the local agency, off-site at a readily <br />available location. <br />
