No invoice numbers <br /> were observed on the consolidated manifests. <br /> All copies of each receipt shall contain all of the following information: <br /> -The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative; <br /> -The date of the shipment; <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes,the waste stream <br /> type listed in subdivision (c), and its proper shipping description, including the hazardous class and United <br /> Nations/North America(UN/NA)identification number, if applicable; <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, address, and identification number; <br /> -The driver's signature; <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator,to be <br /> economically practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain <br /> all required information. <br /> This is a Class II violation. <br /> 203 HSC 25189.5(c)Transported or caused transportation of hazardous waste to an unauthorized point. <br /> The facility services power generator systems at other locations and periodically produces waste antifreeze in the <br /> maintenance process. The waste is then hauled back to 7100 S Longe St#300 in Stockton, CA 95206 and <br /> disposed of as a hazardous waste. <br /> The facility does not have a hazardous waste transporter registration certificate or a variance from the Department <br /> of Toxic Substances Control (DTSC)to transport the antifreeze hazardous waste from the service locations back to <br /> the Longe St address. <br /> Per California Health and Safety Code section 25189.5(c), a facility must be permitted by DTSC to transport <br /> hazardous waste, including antifreeze that has been determined to be hazardous waste by your facility. <br /> Immediately cease transporting hazardous waste until you obtain a hazardous waste transporter registration <br /> certificate or a variance from DTSC. <br /> This is a Class II violation. <br /> FA0014718 PR0521732 SCO01 03/26/2019 <br /> EHD 22-02 Rev.09/22/16 Page 8 of 12 Large Quantity Hazardous Waste Generator OIR <br />
