The staff will remember SIN (Safety, Isolate and Notify). <br />3.1.2. The staff will isolate the spill upon identification. <br />3.1.3. The staff will notify EVS for spill clean-up response by calling EVS <br />directly or calling the Operator to page EVS, excluding Engineering, <br />Grounds Maintenance, Surgery, GI, Laboratory, Pathology, Oncology <br />Pharmacy and Oncology departments. These departments will respond <br />to their own departmental spills. <br />3.1.4. The staff will obtain the Material Safety Data Sheet (MSDS) of the spilled <br />material/waste if the chemical is known. <br />3.1.5. The staff will provide the MSDS to EVS upon their arrival to clean the <br />spill. Engineering, Grounds Maintenance, Surgery, GI, Oncology <br />Pharmacy and Oncology departments will notify EVS to conduct the final <br />clean up the area after the initial spill clean up has occurred. <br />3.1.6. Financial responsibility for large spill clean up will be that of the <br />department that has the spill. <br />3.1.7. Department Manager or Designee will complete a Responsible <br />Reporting Form (RRF) with details of the spill. This report will be <br />completed within 24 hours and submitted to Risk Management. <br />3.1.8. Department Manager or Designee will complete a Spill / Splash / <br />Release Reporting Form EC.Z.056B and submit to Environmental <br />Health and Safety (EH&S) within 24 hours of the spill. <br />3.2. EVS Department Responsibilities: <br />3.2.1. Respond to the spill location immediately, upon notification. <br />3.2.2. EVS will consult with the department staff to obtain information on the <br />spill including: <br />• Name of chemical <br />• Amount if known <br />• MSDS <br />3.2.3. Consult with EH&S if there is any difficulty or question, in determining if <br />the spill is an incidental or emergency size spill. <br />3.2.4. Small spill volumes are as follows: <br />3.2.4.1. Product Incidental Spill Volume <br />Acids Less than '/z cup <br />Bases & Caustics Less than '/2 gallon / 1.9 L <br />Bleach Less than '/2 gallon / 1.9 L <br />Flammable Liquids Less than'/2 gallon / 1.9 L <br />Formaldehyde Less than % cup / 180 ml <br />Glutaraldehyde Less than '/2 gallon / 1.9 L <br />3.2.5. Maintain the appropriate hazardous materials emergency spill kits in <br />strategic locations throughout the facility. Locations of the spill kits are <br />listed in all EVS closets.
