Don personal protective equipment required according to the type of <br />spill. At a minimum, the following should be used: <br />• Nitrile gloves (2 pair — user must always be double gloved). <br />• Chemical resistant gloves (To be worn over double set of nitrile <br />gloves). <br />• Impermeable suits <br />• Safety goggles <br />• Face shield <br />• Surgical mask <br />• Shoe covers <br />3.2.7. Dispose of all spilled clean-up materials/waste according to Hazardous <br />Materials and Waste Management, EC.HZ.039. <br />3.2.8. The EVS Supervisor, Manager or designee will complete the <br />Spill/Splash/ Release Reporting Form EC.HZ. 0566 and submit to the <br />next Hazardous Materials and Waste Workgroup meeting for review. <br />3.2.9. The EVS Supervisor or Manager will complete a Responsible <br />Reporting Form (RRF) with details of the spill. This report will be <br />completed within 24 hours and submitted to Risk Management. <br />3.2.10. The EVS Supervisor or Manager will ensure annual spill response <br />training consisting of written and hands on portion, occurs for all staff <br />members. <br />3.2.11. Department is responsible for maintaining all training documentation <br />and must have it available for any survey or regulatory agency that <br />requests it. <br />3.3. Operator Services Department Responsibilities <br />3.3.1. Upon notification of a spill obtain the following information from the <br />caller: <br />• Name <br />• Extension <br />• Department <br />• Location of Spill <br />3.3.2. Notify EVS, including Lead Aide, Supervisor or Manager if spill occurred <br />inside the building. <br />3.3.3. Notify Engineering or Gardeners for spills occurring outside the building. <br />3.3.4. Notify EH&S. <br />3.4. Plant Operations, Landscaping and Capital Projects Facilities Construction <br />Department: <br />3.4.1. Will be responsible to clean small spills in their department and on the <br />facility grounds. <br />3.4.2. Report all spills to the Chief Engineer, Support Service Leader or <br />Designee. This notified person will then notify the EH&S Department. <br />3.4.3.
