I need some assistance from you in responding to these -itations. <br /> Violation 1: This registered large medical waste generator does not maintain individual treatment <br /> records and shipping and tracking documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain i idividual treatment <br /> operation records, and shipping and tracking documents for all untreated medical waste shipped offsite <br /> for treatment for 2 years. HSC117975(a)-Tracking record for 10/14/21 was incompaete. Contact medical <br /> waste hauler for treatment record and provide a copy to the EHD <br /> Question 1: Attached is the generator portion of this manifest but the destruction portion is not <br /> available. Local staff,Tracey Clark, informed me that she reached out to her local Stericycle contact via <br /> phone at the time and via email following this inspection on 12/28/2022 (I'm waiting on a copy of the <br /> email) but she did not receive a response. Could you provide me with a copy of the destruction <br /> manifest and can you inform me of the process to follow if we are not able to reconcile the manifests? <br /> Violation 2:A person who generates 20 or more pounds of biohazardous waste per month shall not <br /> contain or store that waste above 32 F onsite for more than 7 days.A person who generates less than 20 <br />
