0 <br /> EMPLOYEE TRAINING <br /> 4.1 TRAINING REQUIREMENTS <br /> 4.1.1 ALL EMPLOYEES <br /> All field personnel working-Site who may be exposed to hazardous substances, health hazards, <br /> or safety hazards, and their supervisors and managers responsible for on-Site activities must have <br /> met the following requirements prior to the commencement of Site activities: <br /> • 40-hour HAZWOPER course, AND <br /> • 8-hour HAZWOPER annual refresher course, OR <br /> • Equivalent documented work experience demonstrating a knowledge of safety <br /> and healthful work practices, OR <br /> • A combination of prior training and documented work experience,sufficient to <br /> satisfy the 40-hour minimum requirement and have completed 24-hours of <br /> supervised on the job training. <br /> 4.1.2 SUBCONTRACTORS <br /> Subcontractors on-Site occasionally for a specific limited task (i.e. land or geophysical surveying) <br /> and who are unlikely to be exposed over permissible and published exposure limits shall have <br /> received a minimum of 24 hours of instruction off of the Site, and a minimum of one day actual <br /> field experiences with a trained, experienced supervisor. <br /> 4.1.3 ON-SITE MANAGEMENT <br /> On-Site management and supervisors directly responsible for, or who supervise, employees <br /> engaged in Site operations will have completed a minimum of 8 hours of training in managing <br /> such site operations prior to the commencement of field activities. <br /> 4.1.4 EMERGENCY RESPONSE PERSONNEL <br /> Employees who have been designated as emergency response personnel will have received <br /> additional training in responding to such expected emergencies prior to the start of field <br /> activities. <br /> 4.1.5 NON-TRAINED PERSONNEL <br /> Employees who have not received the required training prior to the commencement of field <br /> activities are not to engage in field activities until such training has been completed. <br /> 13 AW OAWVMM <br /> P:\707 Cove Contractors\Groundwater\Well Installation Workplans\SHSP\SHSP_FINAL.doc <br />
