B/aes Environmental Management Inc <br /> 002-70950 03 U-Hau[#769-50 <br /> Work Plan <br /> 11/8/01 <br /> upon the neighboring property owner and issues they may have with granting access to their property It <br /> is anticipated that obtaining off-site access agreements may take up to three months <br /> 3 12 SJC Well Installation Permittm <br /> Prior to scheduling drilling activities, Blaes Environmental will apply for well installation permits from <br /> the SJC Upon approval of this work plan, Blaes Environmental will prepare and submit permit <br /> applications to drill and install the proposed wells <br /> 3 1 3 Underground Utility Investigation <br /> Upon receipt of SJC well installation permits, Blaes Environmental will schedule well installation <br /> activities with a drilling contractor As part of drilling activities, prior to drilling, Blaes Environmental <br /> will employ a private utility locating service and contact USA Dig Alert to locate existing underground <br /> utilities at each proposed well location A Blaes Environmental representative will be on site to monitor <br /> private utility locating and verify clearance for proposed well placements <br /> 3 14 Well Installation Activities <br /> Blaes Environmental will contract a California-licensed drilling contractor to install the groundwater monitor <br /> wells Each of the three proposed wells will be constructed with one 2-inch well casing screened 20 feet <br /> above and 20 feet below the current groundwater level A 40-foot screen interval is proposed to <br /> accommodate the fluctuating groundwater level The proposed wells will be constructed as shown on Figure <br /> 12 Well installation activities will comply with regulatory requirements and/or the regulatory well permits <br /> issued for each well During drilling of the pilot hole of the wells, Blaes Environmental will log the soil <br /> cuttings and note any pertinent observations on the boring log sheet(example included in Appendix A) <br /> Final completion of each well will be flush with the ground surface with a traffic-rated well vault A <br /> licensed land surveyor will "tie in" each well with the existing monitor wells by measuring the well-head <br /> elevation on the north side of each well casing In addition, the longitude and latitude of each new well <br /> will be identified to sub-meter accuracy <br /> Upon installation of the well casing and the sand pack, the drilling contractor will develop each well by <br /> surging and bailing Surging and bailing will settle and clean the sand pack before adding the seal and <br /> grout to the annulus After completion of the wells, further development will involve surging, bailing <br /> 10 <br />
