(Doulos)abandoned monitoring wells,vapor extraction wells and <br /> sparge points,as well as prepared this report for the subject site. The purpose of this work was <br /> to meet the requirements for case closure for the underground storage tank investigation that was <br /> conducted at this site as requested. <br /> FIELD WORK <br /> The work performed included: Obtaining well abandonment permits from San Joaquin County <br /> Environmental Health Department,encroachment permits for MW-18 and MW-18D(permits <br /> enclosed),abandonment of 31wells,and preparing a report which presents field procedures and <br /> volumes of grout used. All activities were completed in compliance with the San Joaquin <br /> County guideline and California State Department of Water Resources Bulletin 74-81. <br /> Well Abandonment <br /> On April 16,2012,Doulos personnel abandoned 15 wells: MW-4, -5, -6, -7, -8,-13,-14, -15, - <br /> 16,-17, -18,-18D,-19, -20,and-21. Prior to pressure grouting the wells,the volume of grout <br /> needed to displace the pore space in the sand pack, and the volume of the casing was calculated. <br /> The wells were abandoned in place by pressure grouting with a neat cement slurry.The grout <br /> was placed with a trimmie hose from approximately five feet above the bottom of the well to the <br /> surface. After the grout had been trimmied to the surface additional grout was added to maintain <br /> the grout level at the top of the casing as it pushed its <br />
