A report of findings detailing the work that <br /> was performed and the results of the analysis of the samples collected has not been <br /> submitted to PHSIEHD for review. Submit the report to PHSIEHD no later than October <br /> 20, 2000. Reports of findings are due to PHSIEHD no later than 60 days following the <br /> completion of each phase of work.
