A designated operator inspection was performed on December 18, 2018 and the next inspection was performed on January 22, 2019, 34 days later. The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure that all designated operator inspections are performed within the required time frames. The designated operator failed to check and note all required items on the visual inspection reports. -The February 2018 inspection report included incorrect monitoring system certification and spill bucket test dates (1/23/17). The correct date was 1/26/18. -The 10/29/18 inspection report left blank the question regarding UDC sensor locations. -The 12/18/18 inspection report did not include the date signed by the owner/operator. -The 1/22/19 inspection report was not signed by the owner/operator. During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. The designated operator failed to document all the alarms from the attached alarm history on the designated operator monthly inspection reports and failed to check that they were responded to appropriately. The missing alarms include: L1 (87 annular) sensor out alarms on 8/28/18 L5 (diesel UDC) sensor out alarms on 3/25/18, 4/13/18, 4/14/18, 4/25/18, 7/29/18, 8/2/18 During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 210 CCR 2716(f) DO monthly reports not maintained on site for 12 months or for 36 months (after October 1, 2018). The June and July 2018 designated operator monthly inspection reports on site were not for this facility. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies of the missing reports to the EHD. This is a minor violation. 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. A fuel leak was observed under the diesel dispenser whenever the pump was running. All primary containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component under permit and inspection of the EHD. This is a minor violation.
