MEDICAL WASTE MANAGEMENT PLAN <br /> HOSPITAL NUMBER: HOSPITAL LOCATION : CA <br /> EPA ID NUMBER: <br /> I . PURPOSE AND RESPONSIBILITIES <br /> The purpose of this Medical Waste Management Plan (" MWMP" ) is to provide guidance and <br /> describe requirements for the proper management of medical waste in Banfield hospitals as <br /> required by the California Department of Public Health, Occupational Safety and Health <br /> Administration, Department of Transportation, United States Postal Service, Drug Enforcement <br /> Agency, and Environmental Protection Agency. <br /> This MWMP shall be maintained onsite at this hospital. It shall be reviewed annually and <br /> updated whenever changes related to medical waste generation or handling occur. It shall be <br /> made available upon request to the medical waste transporter, medical waste treatment facility <br /> and to the California Department of Public Health. All records related to medical waste <br /> management at this hospital shall be maintained at this hospital for at least three (3 ) years . <br /> The Practice Manager or Chief of Staff is responsible for MWMP implementation at this <br /> hospital. This MWMP will be maintained in the Safety Binder. <br /> II . DEFINITION AND IDENTIFICATION OF MEDICAL WASTE <br /> Medical waste potentially generated at this hospital may include : <br /> ® Sharps (needles , scalpel blades, broken microscope blades or cover slips) <br /> Medical waste : used , absorbent materials saturated with human blood, with human <br /> blood products, with other potentially infectious materials ; or animal blood if infectious <br /> to humans ', and such used absorbent materials that have dried . <br /> ® Waste pharmaceuticals <br /> 1www osha govt s / oshaweb / owadiso show documentZy table =STANDARDS&n id= 10051 # 1910 , 1030 b <br /> Page 1 of 9 <br /> i <br /> I <br />
