The designated operator failed to document all the alarms from the attached alarm history on the February 2018, March 2018, September 2018 and December 2018 designated operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarms include: L6-87 STP fuel alarm on 1/27/18, T3-Diese product overfill alarm on 3/1/18, T2-91 product overfill alarm on 8/30/18, T1-87 product overfill alarm on 8/30/18 and 11/28/18. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing inspections within 30 days of the previous inspections at this facility are including all of the required information on the reports. Furthermore in the 10/25/18 DO report under: -Section I The DO failed to write the CERS ID -Section III The DO failed to add a description to all the components marked N/A or N from section VII -Section X The DO failed to mark YES, NO or N/A under the question"Has the line tightness testing been completed within the required time frames?"and""Has the tank tightness testing been completed within the required time frames?" In the 11/19/18 DO report under: -Section III The DO failed to add a description to all the components marked N/A or N from section VII -Section IX The DO failed to mark Yes, No, or N/A to the question"Are all sensors in under-dispenser containment located to detect a leak at the earliest opportunity?" -Section X The DO failed to mark YES, NO or N/A under the question"Has the line tightness testing been completed within the required time frames?"and""Has the tank tightness testing been completed within the required time frames?" This is a repeat violation, Class II. 210 CCR 2716(f) DO monthly reports not maintained on site for 12 months or for 36 months(after October 1, 2018). The April 2018, May 2018, and December 2018 designated operator monthly inspection reports were not found on site. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies to the EHD. The operations manager was able to get the May 2018 and April 2018 inspection report while the inspector was on site, however the December 2018 DO report was not found.
