S A N J O A U I Environmental Health Department <br /> — C)U NTY— <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> F-FD ORCHARDS 21360 E WALNUT DR, LINDEN September 01, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> The site map was not submitted in 2017, 2018, 2019 and 2020. The submitted historical site map was <br /> lacking loading areas, storm and sewer drains, emergency shut offs and evacuation staging area. A site map <br /> shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and <br /> storage areas, and emergency response equipment. If a site map element is not applicable for your facility <br /> then list it on the map and label as"NA". This was corrected on site. <br /> This is a minor violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the HMBP is complete and accurate by the due <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental <br /> Reporting System(CERS) annually(no submittals were made in 2017, 2018, 2019 and 2020). The <br /> hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br /> beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, <br /> enter the correct or updated information for the elements which were not corrected at the time of inspection <br /> due to lack of information, and submit to the EHD for approval. Provide proof of correction to the San <br /> Joaquin County Environmental Health Department(EHD)within 30 days. <br /> This is a Class II violation. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Mr. Fredrick Debenedetti indicated that facility employers don't receive appropriate initial and annual <br /> refresher training. The business plan shall include provisions for ensuring that appropriate personnel receive <br /> initial and annual refresher training. All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material, including, but not limited to, familiarity with the <br /> following plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the <br /> unified program agency. <br /> (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or <br /> damage to persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice, for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a <br /> minimum of three years. <br /> Immediately provide employee training for appropriate personnel and submit a copy of the training records <br /> to the EHD. <br /> This is a Class II violation. <br /> FA0005157 PR0525874 SCO01 09/01/2020 <br /> EHD 19-01 Rev.8/11/2020 Page 3 of 5 Hazardous Materials Program OIR <br /> 1868 E.
