For all other releases too large or beyond the scope of Hazard Communication training and specific departmental <br />training, notify the Verisk 3E (1-800-451-8346) to have an off-site contractor dispatched for professional assistance <br />to clean up and dispose of hazardous material. Emergency response services (i.e. fire department) will be notified <br />and evacuation procedures will be implemented as the situation warrants. 3E will advise Costco Environmental <br />Services/Hazardous Materials and the location is to notify their Regional Operations, <br />Emergency Notification Procedures: <br />In the event of a reportable release or incident, the Warehouse Manager or Manager on Duty will notify emergency <br />response services, the Verisk 3E (who will notify Costco Environmental Services/Hazardous Materials), and <br />Regional Operations. If evacuation or emergency notification is necessary, employees and customers will be notified <br />via the bull horn and verbal means. Cell phones and portable radios can be used to notify employees <br />neighboring facilities as the situation warrants. If necessary, initiate warehouse evacuation <br />If an unforeseen explosion occurs, Costco employee shall take the following steps: <br />1. If necessary, initiate warehouse evacuation <br />C) Evacuation* <br />The Warehouse Manager or Manager on Duty will coordinate evacuation activities to assure member and employee <br />safety. Employees shall provide assistance for physically handicapped employees, members, visitors, or vendors <br />who are unable to leave the building or area. The decision to evacuate any Costco Warehouse will rest with the <br />Warehouse Manager and the following procedures will be followed. <br />1) Signals that will be used to initiate evacuation include announcements with a bull horn <br />, verbal means <br />and via cell phones/portable radios (Nextel) that management personnel utilize. <br />2) Evacuation will occur through the nearest emergency exits that are located throughout the perimeter of <br />the building. Exits are clearly marked with self -illuminating signs. The evacuation staging area is in <br />front of the building in the parking lot. <br />3) In the event that any primary emergency exits are blocked due to a release of hazardous waste or fire, <br />the nearest alternative emergency exit will be utilized to ensure safe evacuation. <br />D) Sustained Actions: <br />In the unlikely event that a spill would require a prolonged clean-up procedure, Costco will contact Verisk 3E to <br />request a prescreened local licensed emergency response contractor. <br />G <br />January 2019 <br />
