The following is an itemized list of hazardous waste violations that have not been <br /> addressed for GILL EXPRESS INC as of September21, 2018. <br /> Open violations from August 17, 2018 inspection <br /> Violation#303 -Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> The fire extinguisher in the quick lube area at the southwest corner had the indicator pointed to recharge. All facility <br /> communications or alarm systems, fire protection equipment, spill control equipment, and decontamination <br /> equipment, where required, shall be tested and maintained as necessary to assure its proper operation in time of <br /> emergency. Immediately begin this practice. <br /> Violation#403 - Failed to keep hazardous waste containers closed except when adding or removing <br /> hazardous waste. <br /> -The filter crusher and used filters are drained into a spill pallet. This spill pallet is not emptied daily per supervisor <br /> and does not have a proper lid. <br /> -One black metal 55 gallon drum of used paper filters was observed in the quick lube area without a lid <br /> -Three black metal 55 gallon drums of paper filters were observed in the outside hazardous waste storage area <br /> without the lids securely fastened. <br /> All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately <br /> close these containers and ensure all hazardous waste containers are closed when not adding or removing waste. <br /> Violation#502 - Failed to comply with new hazardous waste tank system assessment requirements. <br /> Facility does not have a tank assessment for the used oil tank. The owner or operator of a hazardous waste tank <br /> shall obtain a written assessment reviewed and certified by an independent, qualified, professional engineer, <br /> registered in California attesting that the system has sufficient structural integrity, is acceptable for the transferring, <br /> storing, and treating of hazardous waste, and that the tanks and containment system are suitably designed. <br /> Immediately have an assessment prepared for the used oil tank. Submit a schedule showing when this will be <br /> accomplished to the EHD and the completed tank assessment. <br /> Violation #507 - Failed to conduct daily inspections of hazardous waste tank system and maintain records <br /> onsite. <br /> Records of daily tank inspections were not available for review at the time of the inspection. Inspections of the <br /> hazardous waste tank system was not performed and documented. The following items are required to be <br /> inspected at least daily: <br /> 1. overfill/spill control equipment; <br /> 2. aboveground portions of the tank system to detect corrosion or releases of water; <br /> 3. data gathered from monitoring and leak-detection equipment; <br /> 4. outside areas of the tank system including secondary containment to detect erosion or signs of releases of <br /> hazardous waste; <br /> 5. for uncovered tanks, the level of waste in the tank. <br /> Immediately begin required inspections of the hazardous waste tank system, and document the items inspected. <br /> Submit a copy of the inspection log for the next 30 days to the EHD. <br /> Page 3 of 7 <br />
