Hazardous Waste Management <br /> HAZARDOUS WASTE PICK-UP PROCESS <br /> DAY OF HAZARDOUS WASTE PICK-UP <br /> 1) Stericycle will remove all the hazardous waste from the totes in the Accumulation Area. <br /> 2) The Technician will sort and weigh all of the Hazardous Waste and repackage it for transport. <br /> 3) The Technician will remove the labels and return the empty totes to the Hazardous Waste <br /> Accumulation Area. (MOD must ensure that all totes are empty and the labels have been removed) <br /> 4) The Technician will fill out a Uniform Hazardous Waste Manifest; listing total weight of each type of <br /> Hazardous Waste is being picked up. <br /> 5) MOD will confirm the total weight for each hazardous waste type is correctly listed on the manifest. <br /> 6) The Technician will sign and date the Uniform Hazardous Waste Manifest on behalf of 99¢ Only <br /> Stores in the Generator section. <br /> 7) The Technician will also sign and date the manifest in the Transporter section confirming he is taking <br /> possession of the waste listed on the Uniform Hazardous Waste Manifest. <br /> 8) The Technician will leave an initial copy of the Uniform Hazardous Waste Manifest with the MOD. <br /> This copy must contain signatures in the Generator and Transporter sections. <br /> 9) The Uniform Hazardous Waste Manifest (with Generator and Transporter signatures) should be filed <br /> in the Hazardous Waste Management Binder pending the final copy. <br /> WITHIN 45 DAYS OF HAZARDOUS WASTE PICK-UP <br /> 1. The TSDF (Treatment, Storage and Disposal Facility) will mail the FINAL Manifest to the store. <br /> ✓ The FINAL Uniform Hazardous Waste Manifest will have an additional signature confirming <br /> the Hazardous Waste was received by the TSDF (Treatment, Storage and Disposal Facility). <br /> 2. Store must staple the FINAL Uniform Hazardous Waste Manifest to the front of the initial manifest <br /> (given to the MOD when the Hazardous Waste was picked up). The manifest is now completed. <br /> 3. All manifests must be kept in the Hazardous Waste Management Binder for a minimum of 3 years <br /> and must be made available to an inspector upon request. <br /> If you do not receive a FINAL Uniform Hazardous Waste Manifest within 45 days from the pick-up date, <br /> contact PARM (855) 784-3999. <br /> Confidential Information.
