OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. The DO report of July 2021 was not found on site during the inspection. REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a minor violation. 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -The DO report of 5/27/2021 and 6/23/2021 did not include the owner/operator name, signature and date. -The DO report of 02/03/2021 failed to mention a fuel liquid alarm L4 (UDC 7/8)occurred on 01/28/2021. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a minor violation. 211 CCR 2715(c)(2) Failure to have or implement a training program approved by the UPA, at an unstaffed facility. OBSERVATION: The designated operator employee training for 20/21 was not performed or current. REGULATION GUIDANCE: For facilities that are not routinely staffed,the designated operator shall implement an employee training program approved by the EHD. CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. This is a Class II violation.
