Results of all self-monitoring programs as described in the Report of Facility Information, will be reported as follows: <br />Program <br />Reporting Facility <br />Agency Reported To <br />1. Log of Special Occurrences (i.e. tires, explosions, <br />ANNUALLY <br />LOCAL ENFORCEMENT AGENCY <br />accidents, inadvertent acceptance of hazardous wastes, <br />(LEA) <br />etc...) must be kept on site and submitted to LEA annually. <br />2. An employee training log with course descriptions, dates of <br />KEEP ON SITE AND AVAILABLE <br />LEA <br />training, etc... shall be maintained and kept current. <br />UPON REQUEST OF LEA <br />3. A record of weather conditions shall be maintained with <br />UPON REQUEST OF LEA <br />LEA <br />operational records pursuant to 14 CCR Section 17875. <br />4. A report of quantities and types of wastes received and <br />product removed from the facility shall be provided to the <br />QUARTERLY <br />LEA <br />LEA on a quarterly basis (showing monthly breakdowns). <br />The records shall indicate the quantities in tons/cubic yards <br />and include weight and record data. Records shall be <br />submitted to the LEA on the 15th day of the subsequent <br />month. <br />5. Temperature of the active compost shall be monitored and <br />recorded as specified in the California Code of Regulations, <br />UPON REQUEST OF LEA <br />LEA <br />Title 14, Division 7, Articles 3 & 4 (14CCR). Records <br />shall be kept for kinds and amounts of additives/ <br />amendments used in composting process. Whatever <br />composting method used all records pertinent to the <br />particular operation are required to be kept current on site <br />as per 14CCR Section 17887. <br />6. In addition to storm water monitoring records a log shall be <br />kept for leachate monitoring. One sample of leachate shall <br />QUARTERLY <br />LEA, Regional Water Quality Control Board <br />be taken from each of the first five composting batches of <br />(RWQCB) <br />feedstock generating leachate and analyzed for organo- <br />phosphates, using EPA method 8140, plus nitrate testing <br />(14CCR, Section 17876.(A)1) <br />7. One sample per 5000 cy of finished compost must be taken <br />and analyzed at a Department of Health Services Approved <br />MONTHLY <br />LEA <br />Laboratory. Samples shall be analyzed for constituents <br />required pursuant to 14 CCR, Section 17887. <br />8. A certification that pathogen reduction criteria are met must <br />be completed using CIWMB Form 5000. <br />LEA <br />9. Submit an annual report to the LEA by January 31, of each <br />ANNUALLY <br />calendar year summarizing all reporting requirements. <br />
