Complaint Investigation Summary <br /> Report #. 5105 <br />COMPLAINT ID: COQSOONYP Site Location: SOUTHBOUND HWY 5, SOUTH OF DURHAM FERRY RD <br />Activity Summary <br />Activity Date Recorded by <br />11/13/2020 Received a call from Brandon Espinoza with All Environmental Services asking for ALANIZ <br />the RP information of an adjacent spill (50 feet away). Informed Brandon to call <br />the lead inspector for the complaint back on Monday. Forwarded the information <br />to the inspector. <br />11/19/2020 Received emails from Brandon Espinoza of All Environmental and Juan FLORIDO <br />Armendarez of Caltrans regarding possibly scheduling the final clean-up to begin <br />on 11/23/20 at 9am. <br />11/19/2020 Emailed CUPA ER staff the details of this complaint, so that they may verify the FLORIDO <br />complaint clean-up during my absence. <br />11/23/2020 Reviewed ER file and photo. GAITAN <br />11/23/2020 Sent message to Brandon Espinoza, owner, All Environmental. Asked if clean up GAITAN <br />was still scheduled for today at 9am. He stated he was heading over to spill area <br />and would give me an update once he gets there. <br />11/23/2020 Received call from Juan Armendarez, HazMat Supervisor for Caltrans. Stated <br />clean up is nearing completion. Arrived on site, met with Brandon Espinoza, owner <br />of All Environmental and Juan. I observed the following: <br />-Approximately 4 yards of soil impacted with diesel generated from clean up. <br />All Environmental crew continued excavation until heavy diesel stains and odor <br />wore no longer visible. I observed no heavy diesel stain and no odor. Discussed <br />clean up with Juan, he stated clean up was good for him. I determined clean up to <br />be adequately cleaned. Asked Brandon how he was managing the waste, he <br />stated that the soil impacted with diesel would be taken as non-hazardous waste. <br />Asked how he made a waste determination, he cited generator knowledge and <br />stated composite sampling would be provided to me. Contacted Elena Manzo <br />'Lead Senior REHSI discussed clean up. Based on discussion, I informed Brandon <br />if the results for soil impacted with diesel comes back as hazardous waste, then <br />he would have transferred hazardous waste without a manifest and proper <br />labeling. He stated that he understood. <br />Left site. <br />GAITAN <br />11/24/2020 Reviewed complaint with Melissa Nissim 'Program Coordinator'. GAITAN <br />11/25/2020 Generated photo log, sent photo log to Elianna Florido 'REHS'. GAITAN <br />11/30/2020 Reviewed Jason Gaintan's, REHS, photo log. Discussed complaint with Jason. All FLORIDO <br />Environmental disposed of excavated soil as non-haz, via generator knowledge, <br />and will provide sampling results and disposal record. <br />11/30/2020 Corrected errors on photo log, sent updated copy to Elianna Florido IREHSI. GAITAN <br />12/16/2020 Reviewed narrative. Emailed Brandon Espinoza of All Environmental requesting FLORIDO <br />he provide the waste sample analytical results and waste disposal record. <br />01/07/2021 Emailed Brandon Espinoza of All Environmental, and CC'd FLORIDO <br />subedartrucking©gmail.com, the responsible party, requesting he submit the <br />waste sample analytical results and disposal record from this spill clean-up, in <br />order to close out the complaint for Subedar Trucking (responsible party). <br />01/12/2021 Reviewed and discussed mistake in billing with Elena Manzo, Ld Sr REHS, and FLORIDO <br />Michelle Ozuna in accounting.
