6.2 Permitting <br /> 6.2.1 Monitoring Well Permits <br /> Monitoring well permits will be obtained from the San Joaquin County Environmental Health <br /> Department(EHD). Rubik will notify the EHD and RWQCB at least 48 hours prior to the field work. <br /> 6.2.2 Notice of Intent for Injections <br /> Upon approval of this work plan, a Notice of Intent (NOI) will be submitted in accordance with <br /> RWQCB General Waste Discharge Requirements(WDR)Order No. R5-2015-0012.The RWQCB <br /> indicated no bench scale testing will required because the injectants have been used elsewhere <br /> in California for the same purpose. Additionally, the injectants are approved in the California <br /> RWQCB — Los Angeles Region Waste Discharge Requirements for In-Situ Groundwater <br /> Remediation (Order No. R4-2014-0187). <br /> 6.3 Health and Safety Plan <br /> A site-specific Health and Safety Plan (HASP) will be reviewed and signed by all field personnel, <br /> including subcontractors, prior to work initiation. The HASP will identify potential health and safety <br /> hazards for each phase of site work and include requirements and procedures for protection. The <br /> HASP will be maintained onsite during the field activities. <br /> 6.4 Utility Clearance <br /> Underground Service Alert(USA)will be notified a minimum of 48 hours in advance of subsurface <br /> activities to identify underground utilities. A private utility locator will also perform a geophysical <br /> survey in the vicinity of each well and injection location to identify potential utilities or other <br /> subsurface obstructions. If facility personnel, data from previous borings, and/or the utility <br /> clearance contractor are unable to verify a location is utility free, a hand auger will be advanced <br /> to approximately 5 feet bgs prior to drilling to manually clear the location. <br /> 6.5 Monitoring Well Installation <br /> The proposed monitoring well locations are depicted on Figure 8. The wells will be installed using <br /> a truck-mounted hollow stem auger drilling rig. During drilling, soil cores will be collected <br /> continuously and logged in the field using the Unified Soil Classification System (USCS). The <br /> monitoring wells are anticipated to be screened between 45 and 65 feet bgs to be consistent with <br /> the surrounding A-zone wells. After reaching the maximum extent of drilling,the conductor casing <br /> will be removed in stages as the annular space is backfilled with the well construction materials. <br /> The monitoring wells will be constructed of 2-inch diameter Schedule 40 polyvinyl chloride (PVC) <br /> casing with 20 feet of 0.010-inch slotted screen. The annular space between the borehole wall <br /> and the well screen will be back-filled with a silica sand filter pack to approximately 2 feet above <br /> the top of the well screen. A 2-foot thick hydrated bentonite pellet seal will be installed above the <br /> filter pack.
