DMR'S required by the California Regional Water Quality Control Board <br />Data required by the California Department of Health <br />Overview statement of operation and any significant events <br />Staff Changes <br />Monthly emergency call-out reports <br />Record of staff safety meetings <br />Synopsis of all regulatory reports submitted by 7-H <br />Monthly sewer cleaning and inspection reports <br />Progress report toward completion of annual budget items <br />Identification of new problems <br />Cumulative total of repair fund accounts expenditures to date <br />Storage Pond Capacity Elevations <br />Time Sheets with invoice <br />Equipment operations logs <br />A summary Annual Report (three copies) shall be prepared at year end upon <br />request of the Director of Public Works. <br />3.9 Sewer Collection System. 7-H will be responsible for the inspection and <br />cleaning of the gravity sewer lines and manholes (as defined in ATTACHMENT <br />"A") and as scheduled herein. 7-H shall inspect the general area of location of <br />force mains once each month for leaks. 7-H shall inspect gravity sewer mains <br />that are not accessible with cleaning equipment. <br />On a monthly basis 7-H shall hydro-clean a minimum of 15,000 lineal feet per <br />month of main and collector sewer lines, and related manholes until the entire <br />system is cleaned. 7-H shall document by monthly reporting the reaches and <br />physical lengths that have been cleaned for any given month of the year that <br />cleaning is performed. 7-H shall clean the "reaches" of the CITY'S "hot spots" at <br />least once a month. 7-H shall place certain areas of the sewer system on the <br />"hot spot" list when stoppages occur more than two (2) times in any given <br />monthly period. Such new additions to the list shall be reported to the CITY in <br />the Monthly Sewer Report Section of the Monthly Report. 7-H shall have access <br />and use of the CITY owned "Aqua-Tech" hydro vacuum cleaning equipment. 7-H <br />shall be responsible for damage caused by 7-H through the use by 7-H. 7-H <br />shall provide liability insurance, fuel, oil and supplies for the "Aqua-Tech" during <br />7-H's use and shall be responsible for maintenance repairs on the "Aqua-Tech" <br />up to dollars per contract year. 7-H shall provide the <br />CITY with proof of proper licensing and driving records of its personnel assigned <br />to the "Aqua-Tech" unit. <br />7-H shall clean all seven (7) pumping station wet wells once every ninety (90) <br />day period or earlier as needed. <br />3.10 Collection System Access. 7-H shall, in lateral sewer areas determined to <br />have location access problems, utilize hand rods or flex snakes to clear <br />7-H Technical Services Group Contract for Operation and Maintenance Services Page 6 of 12
