Subcontractor employers are ultimately responsible for ensuring that their personnel are adequately protected
while working on a Stratus Environmental project site. In signing this agreement, a subcontractor acknowledges
that:
They are responsible for the implementation of all FIASP requirements
They are responsible for communicating the information regarding safety or health hazards presented in
this HASP to their employees who will be on site.
Should any of their employees act in a manner indicating a lack of understanding of the environmental,
health and safety hazards on the site, they may be asked to attend more training and/ or leave the job site.
Prior to entering the work area, this HASP must be reviewed by all personnel planning to enter and/or perform
work on this project site. Acknowledgment that the employee has reviewed and understands the hazards
identified shall be indicated by signature on the HASP acknowledgement form, located at the end of the HASP.
This includes ALL Stratus Environmental and subcontractor employees, client representatives, vendors and
visitors.
BP Arco # HASP Created by: Scott Bittinger Revised by: Evan Lee Fau
Template Version: 02.02 Created on: 10/02/2017 Revision date:10/30/2017
