1.7 INCIDENT REPORTING PROCEDURES <br />INSTRUCTION GUIDE FOR INTERNAL INCIDENT REPORTING <br />FOR THE STANTEC CHEVRON TEAM <br />This section outlines the procedures that will be followed in the event of an incident. A flowchart and a <br />table with necessary contact information (phone numbers, fax numbers, and email addresses) for incident <br />reporting are also provided. <br />In the event of an incident (injuries, spills, non-conformance events, permit exceedances, and potential <br />incidents (near losses)): <br />1. Stop work, stabilize the situation, secure the site and notify the SHSO. If you are unsure whether <br />or not something should be reported, Stop Work and proceed with notification anyway. <br />Employees Requiring Medical Affention: <br />For life threatening injuries, immediately contact 911. The injured employee will be <br />accompanied by the SHSO to the medical facility. <br />When an employee requests medical attention for a non- life threatening injury, <br />and after alerting the Stantec Project Manager; the employee should immediately <br />call Melissa Helton, Stantec's USWC Claims Coordinator at 513-720-3706 for <br />assistance. <br />The employee will be taken to the approved clinic that is identified in Section 1.0 for <br />first treatment care. <br />The SHSO will make VERBAL notification to the Stantec Project Manager immediately (within the <br />hour). If verbal notification can't be made to the Stantec Project Manager, verbal notification <br />must be made to one of the Stantec Chevron Incident Review Team Members. <br />The Stantec Project Manager will make internal notifications* to the following: and <br />Stantec H&S Team Members (Jon Lessard or Carol Ferguson-Scott and one of the RSECs <br />(Clint Reuter, Keith Kuhlmann, or Fred Miller). Note: Incidents involving injury, potential injury, or <br />report of pain, soreness, or discomfort requires immediate notification to a Stantec H&S Team Members <br />to develop a plan for assessment and care. <br />At least one of the Stantec Chevron Incident Review Team Members (Jim Burns, Brent <br />Lucyk, Tom Madsen, or Bob Wilson). <br />If a team member does not answer, leave a detailed message with a number at which you <br />can be reached. You must make immediate verbal notification to one of the Chevron <br />Incident Review Team Members. If there is no answer, leave a detailed voicemail and <br />continue calling contact numbers for the Chevron Incident Review Team until verbal <br />notification is made. <br />*Internal notification is a CEMC requirement for our Stantec team members. Internal reporting <br />requirements were established for two primary reasons: (1) provide a pathway for employees <br />to obtain the assistance of company health and safety experts during an incident or near loss, <br />and (2) to help Stantec track injuries and near losses that occur to our employees. The data <br />captured during the reporting process is then used to identify trends that can be pro-actively <br />addressed to improve overall health and safety within our company. This helps everyone go <br />home safely. <br />The Stantec Project Manager will obtain concurrence from at least one of the Stantec Chevron <br />Incident Review Team Members regarding CEMC reporting requirements.
