• Proper precautions to take as a result of the release <br />• Name and phone number of person to be contacted for further information. <br />In addition, as soon as practicable after a release, a written follow-up shall be provided that <br />includes updated information on the above items and also includes what actions were taken in <br />response and to contain the release and any known or anticipated health risks. <br />5. IMPLEMENTATION OF THE CONTINGENCY PLAN <br />The decision to implement the contingency plan depends upon whether or not an imminent or <br />actual incident could threaten human health or the environment. The purpose of this section is to <br />provide guidance to the Emergency Coordinator in making this decision by providing decision- <br />making criteria. <br />NOTE: A Triple I Form must be completed for all releases of Hazardous materials. (See VWNA, <br />Environmental Incident — Internal Reporting SOP) <br />The Contingency Plan will be implemented in the following situations: <br />Fire and/or Explosion <br />a. Afire causes the release of toxic fumes. <br />b. The fire spreads and cold possibly ignite materials at other locations on site or could <br />cause heat — induced explosions. <br />c. The fire could possibly spread to off-site areas. <br />d. Use of water or water and chemical fire suppressant cold result in contaminated runoff. <br />e. An imminent danger exists that an explosion could occur, causing a safety hazard <br />because of flying fragments or shock waves. <br />f. An imminent danger exists that an explosion cold ignite other hazardous waste at the <br />facility. <br />g. An imminent danger exist that an explosion could result in release of toxic material. <br />h. An explosion has occurred. <br />Spills or Material Release <br />a. The spill could result in release of flammable liquids or vapors, thus causing afire or gas <br />explosion hazard. <br />b. The spill could cause the release of toxic liquids fumes. <br />c. The spill can be contained on site, but the potential exists for ground water <br />contamination. <br />d. The spill cannot be contained on site, resulting in off-site soil contamination and/or <br />ground water contamination. <br />6. EMERGENCY RESPONSE PROCEDURES <br />• Any person observing, being involved with, or recognizing a discharge, release or spill of <br />hazardous waste is responsible to: <br />• Identify the problem to the best of his ability. <br />7 <br />
