Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: COWPTSQMU Site Location: 10749 W WOODBRIDGE RD <br /> Activity Summary <br /> Activity Date Recorded by <br /> 03/26/2021 5:02 PM En route to spill location. MURO <br /> 5:48 PM Arrived. Met with Chris Feathers (Patriot Environmental Services), Wes <br /> Serpa (Location Manager at Wilbur Ellis), and Trevor Carlson (Property Leaser) <br /> and discussed the spill incident. According to Wes Serpa, the poly tank that <br /> contained the UAN 32 Urea Ammonium Solution was an older tank. It was noted <br /> on video footage that the tank began with a pin-hole leak and failed shortly after. <br /> Wes did not have a tank ID number, but was able to provide the tank's serial <br /> number: 19814574. Conducted a walk through of the area. Observed the large <br /> spill area had impacted farm equipment. The spill reached in close proximity of the <br /> olive tree berm. According to Chris Feathers, the impacted soil spanned an area <br /> of 9,500 square yards and was saturated between 2 -6 inches in depth. Chris <br /> Feathers initiated a USA dig notification for excavation as needed. <br /> Chris Feathers stated that Trevor Carlson had notified California Department of <br /> Fish &Wildlife (CDFW), but did not have any contact information. Regarding the <br /> impacted equipment, plan is to clean using simple green and water. Waste <br /> generated from cleanup of equipment such as rinsate will become part of the soil <br /> waste. <br /> Regarding impacted soil, plan is to do a light scraping of about 12 inches upon <br /> USA dig clearance. Waste will be kept onsite pending Analytical lab for a waste <br /> determination. <br /> Began Notice to Abate. Was having issues bringing up complaint. <br /> 7:06 PM Received page from SJGH. Called hospital dispatch, 209468-6000. <br /> Operator asked me to call Blake Tyson at 916-845-8991. <br /> 7:23 PM Called Blake Tyson; no response. Left message. <br /> Cal OES spill update was emailed to ER Phone Update was: several storm drains <br /> were impacted. <br /> During my first walk-through, I did not see any storm water drains. Did a second <br /> walk with Chris Feathers. Observed south and west drains were impacted. Chris <br /> did not have a plan for remediation of the impacted drains; asked EHD to advise. I <br /> told Chris it will need to be addressed, I will ask a supervisor and get back to him <br /> the following morning. <br /> 7:59 PM Left spill location. <br /> 8:41 PM Arrived home. <br /> Updated spill location address to complaint. Generated site map image. Entered <br /> narrative. <br /> Completed Notice to Abate, emailed to Ernie Spencer <br /> (espencer@spillcenter.com), Wes Serpa wserpa@wilburellis.com, and Trevor <br /> Carlson trevordcarlson@gmail.com. <br /> 03/26/2021 Received new complaint information from REHS Elianna Florido (EHD).
