FOR REFERENCE ONLY BUSINESS CONFIDENTIAL <br /> Hazardous Materials Spill Response HAZ-006 <br /> Damaged, Leaking Hazmat Packages <br /> 4. Once the material is identified, obtain the SDS for the leaking material. <br /> If you are not able to obtain the SDS from the shipper, the product manufacturer should be able <br /> to supply the document. Refer to the inner container label for manufacturer contact information. <br /> 5. Verify you have the correct SDS. <br /> Note: Certain materials such as FedEx Ground prohibited hazardous materials, <br /> biological material, liquid mercury metal, radioactive materials and explosives require <br /> cleanup by a vendor. Contact the FXG Emergency Spill Response Service at <br /> 833.FXG.SPIL (833.394.7745). Refer to the Hazardous Materials Prohibited/Acceptable <br /> Labels Poster, 20175S, for more information. <br /> 6. Obtain the appropriate personal protection equipment and other supplies necessary to safely <br /> handle and clean up the leaking package. <br /> 7. Place the package into a lined, red hazardous material tote for transport back to the hazardous <br /> material processing area. <br /> 8. If the damaged package contains a reportable quantity (RQ marked anywhere on paperwork or <br /> package itself), immediately contact Safety, PGH at 412.859.5960. If there is no answer at this <br /> number, contact your safety specialist. <br /> 9. Complete the Hazardous Material Incident Report within the Hazmat Incident Reporting System, <br /> a hazardous material damaged package report, and a Hazmat improper shipping form (ISF) <br /> with photographs. <br /> 10. Complete a Hazmat ISF if the text on the barcode is not readable. <br /> 11. Get the tracking ID of the package if possible. <br /> 12. Scan the barcode with the MC55 to complete a Hazmat ISF. <br /> The scanner provides the tracking ID number, which is used to obtained the rest of the shipper <br /> information. <br /> 13. Use the photo function on the scanner. <br /> You must take at least three pictures; however, you can take up to five. Ensure that the scanner <br /> is in a well-lit area and allow for the camera to focus with the light to shine if needed. The <br /> pictures should show the outside of the package, all labels and markings (including the barcode <br /> label), any damage to the outside and/or inside of the package or contents, and the barcode if it <br /> is damaged and not scannable. <br /> The Automated Work Area Design (AWAD) load labels also display the tracking number. <br /> Information about the shipper can also found if there is a packing list attached to or inside of the <br /> package. <br /> 14. Wait for disposition from Safety, PGH. <br /> 15. Follow the instructions on the disposition carefully. <br /> The disposition usually indicates that the undamaged portion of the shipment should be <br /> returned to the shipper (RTS). The damaged portion and any cleanup material are sent to the <br /> hub for evaluation, except at California stations where the damaged portion must be managed <br /> as hazardous waste at the facility where discovered.
