SAN)J DAIJ I N Environmental Health Department <br /> sr <br /> C 0 U NTY <br /> Time In: 9:30 am <br /> Greatness grows here. <br /> Time Out: 10:12 am <br /> COMPLAINT INSPECTION REPORT <br /> Name of Facility: WINCO FOODS#10 Date: 04/07/2022 <br /> Address: 5110 MONTAUBAN AVE, STOCKTON 95210 <br /> Owner/Operator: WINCO FOODS LLC Telephone: (209)954-1360 <br /> Program Element: 1600- FOOD PROGRAM Complaint#: C00054795 <br /> Responsible Party Contact: <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> ON 4/4/2022,AROUND 10 AM, COMPLAINANT WAS AT THE FACILITY IN THE COOKIES AND CRACKERS AISLE AND <br /> OBSERVED AN ADULT COCKROACH CRAWLING AROUND ON ONE OF THE SHELVES. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe.
