Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COJINF1PF Site Location: NORTHBOUND HVVY 5, SOUTH OF DURHAM FERRY RD <br />Activity Summary <br />Activity Date <br />11/24/2020 <br />Recorded by <br />8:15 AM Received text from Juan Amendarez (Caltrans) notifying that the spill MURO <br />cleanup on northbound 1-5 has begun at 9:00 AM Called Juan Armendarez to give <br />us one hour's notice of cleanup completion. <br />11:41 AM En route to spill cleanup with CUPA Program Coordinator, Melissa <br />Nissim (EHD). <br />12:16 PM Arrived to spill site cleanup. Met with Jason Morgan (Project Manager <br />with Ancon Services) and Juan Armendarez. <br />Observed excavated area to be approximately 20 X 40 feet. The shallow depths <br />were approximately 5-6 inches deep with the deepest point being approximately <br />4.5 feet deep. Observed blue gloves in dispersed in different areas. Observed <br />Ancon to be using the blue gloves while reading the hydrocarbon levels in the soil. <br />Ancon placed soil inside then the PID meter tip. Purpose being for isolating the soil <br />and PID meter tip and shielding from atmospheric air while measuring Volatile <br />Organic Compounds (VOCs) from excavated soil. The gloves were then used as <br />markers in areas that have been cleared down to 0 ppm of VOCs. Observed one <br />area on southern end of excavation that looked to be slightly darker then the <br />surrounding soil. Asked Ancon to measure that area. Ancon took a reading from <br />the darker soil. Verified the PID meter reading to be 0.5 ppm VOC. No oily soil or <br />odor was detected. Site was deemed adequately cleaned. In total, three 20 yards <br />bins were generated. Two of the three contained about 17 yards diesel <br />contaminated soil; one of three 20 yard bins contained 10 yards of diesel <br />contaminated soil. Excavated soil will be sampled tomorrow for analytical testing <br />using the CAM 17 method for waste profiling. The first of three 20 yards bins was <br />already taken to storage area. Juan Armendarez was unable to provide the <br />address where the three 20 yard bins containing the generated waste will be <br />stored because he didn't know the correct address. He stated the waste will be at <br />a Caltrans facility located near Manthey Rd and Mossdale Rd pending the <br />analytical testing. I let Juan Armendarez and Jason Morgan know that Elianna <br />Florido (EHD) is still the lead inspector for this complaint, copies of lab reports and <br />disposal records should be sent to her. <br />12:43 PM Left site. <br />1:14 PM Arrived at EHD. <br />11/24/2020 Discussed complaint with Elena Manzo (EHD).
