Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COJINF1PF Site Location: NORTHBOUND HVVY 5, SOUTH OF DURHAM FERRY RD <br />Activity Summary <br />Activity Date <br />12/17/2020 <br />Recorded by <br />Reviewed waste soil analytical, received on 12/16/20 from Jason Morgan, Ancon FLORIDO <br />Services Project Manager. Jason states the waste will be disposed as non <br />hazardous at Potrero landfill. Looked up Potrero Hills Landfill and found on their <br />website that they may require pre-approval for soil that is not clean. I emailed <br />Jason asking him if he has written pre-approval from the landfill. Called landfill and <br />spoke to Renee Fowler, facility representative, who referred me to Joe Lynch for <br />questions regarding the landfill's concentration acceptance criteria is for diesel <br />contaminated soil. Renee emailed me the landfill's guidelines for special waste. <br />Called Joe Lynch and left him a voice message and sent him an email regarding <br />whether or not Ancon Services is approved to dispose of the waste at the landfill. <br />12/17/2020 Additional review of analytical results and the contractor's hazardous waste FLORIDO <br />determination and discussed them with Elena Manzo, Ld Sr REHS. <br />12/17/2020 Emailed narratvie and photo log to Elianna Florido. MURO <br />12/18/2020 Called and spoke to Jason Morgan, Ancon Services Project Manager, who states FLORIDO <br />he submitted the waste analytical and profile to Potrero Hills Landfill and received <br />their approval to dispose of the waste. Jason to email me a copy of the approval. <br />01/07/2021 Emailed Jason Morgan and Jason Meehan of Ancon Services, and CC'd the <br />responsible party representataive Talwinder Singh (talwinderdba@gmail.com), <br />requesting he submit a copy of the pre-approval from Potrero Hills Landfill for the <br />disposal of the waste generated from this spill cleanup, as well as the waste <br />disposal records, in order to close out this complaint. <br />FLORIDO <br />01/07/2021 Reviewed and revised narrative. FLORIDO <br />01/25/2021 Reviewed an email received on 12/18/20 from Sabrina Fountain, All Star HEavy FLORIDO <br />Haul & Towing Manager (ofcmngr7@aol.com), containing a letter from Kumiko <br />Horiuchi, customer service representative for Tracy Material Recovery & solid <br />Waste Transfer, certifying that air fryers, slippers, and garbage were taken to <br />Tracy Material Recovery & Solid Waste Transfer facility on 11/17/20, where the <br />materials were crushed and mixed with other materials, and then would be <br />transported to a local landfill for final disposal. The email also contained a Tracy <br />Material Recovery & Solid Waste Transfer weigh ticket for 2.9 tons of mixed <br />commercial waste on 11/17/20. <br />Emailed Jason Morgan and Jason Meehan of Ancon Services, and CC'd <br />talwinderdba@gmail.com (responsible party representative), requesting they <br />provide the disposal record for the waste generated from the final spill clean-up. <br />01/25/2021 Reviewed and revised narrative. Reviewed, printed, and organized complaint FLORIDO <br />documentation. <br />02/02/2021 Called and spoke to Jason Morgan of Ancon, and requested he provide a final FLORIDO <br />signed copy of the waste disposal records. Jason agreed he would once he is <br />back in his office. Emailed Jason a reminder regarding the disposal records. <br />02/12/2021 Reviewed narrative. Called and left a voice message for Talwinder Singh, FLORIDO <br />responsible party contact, requesting he provide the final waste disposal records <br />for this spill clean-up. <br />02/22/2021 Reviewed final disposal records received on 2/18/20 from Jason Morgan of Ancon. FLORIDO <br />Jason provided three non-hazardous waste manifests each documenting the <br />disposal of 20 yards of non-hazardous soil and rock at Potrero Hills Landfill, <br />totaling 60 yards of non-hazardous soil and rock. <br />5105. rpt Page 4 of 5
